The portal was designed to operate on Salesforce-hosted databases, synchronized with the clients databases using on-demand, batch REST APIs. By leveraging Salesforce Health Cloud, the client could interpret and manage substantial amounts of health data to create actionable, personalized, and timely health signals, ultimately leading to improved clinical and financial outcomes.

Results

The implementation of Salesforce Health Cloud proved to be highly effective for the client, delivering significant positive results. The program successfully enrolled over 328,000 members, while 1253 clients launched with the platforms support. Notably, there was a remarkable 100% year-over-year increase in members, while the client also launched an IPO, signaling the success of the program.
